If you live in Kenosha or Racine Counties, please call the Wisconsin Kenosha Racine Partners (WKRP) at 1-888-794-5820 or access the website, access.wi.gov Wisconsin residents can use the ACCESS web site to: - See if they might be eligible for FoodShare, Medicaid/BadgerCare Plus, Medicare part D, Wisconsin Works (W2), SeniorCare, WIC, school summer food programs, emergency food assistance or other programs they might be eligible for. - Apply online for FoodShare, Medicaid, BadgerCare Plus, and the Family Planning Waiver Program - Get up to date information about the status of their FoodShare, Medicaid/BadgerCare, Family Planning Waiver Program, SeniorCare, or Caretaker Supplement benefits. - Get replacement Forward card - Report a job change online - Get answers to common questions about benefits - New ebtEDGE Mobile App for Viewing FoodShare Account Information Members receiving FoodShare benefits can now view information about their FoodShare account using the ebtEDGE mobile app. Members can manage their healthcare, food, and Wisconsin Works benefits online through their ACCESS account or through the MyACCESS mobile app. Can be used to understand how to use the ACCESS app, support page, and flyer: https://www.dhs.wisconsin.gov/forwardhealth/partners/benefits.htm *Call the Quest Card Customer Service hotline: (877) 415-5164 if your FoodShare card is lost, stolen or damaged, balance inquiry, to change your PIN or have other problems or questions with your FoodShare card. **Fax documents (Verifcatons) to CDPU: 1-855-293-1822 OR Mail documents to: CDPU P O Box 5234 Janesville, WI 53547-5234 ***Contact Member Services at 1-800-362-3002 for questons about suspended benefits due to incarceration.*** MA Resources
