Program Description:
Allay Hospice is a hospice provider, who offer a very personalized, team approach to care. Services are available, 24 hours a day, seven days a week providing care at home, assisted living or skilled nursing facilities. Services include: - Dedicated Continuous Care Team to provide care at end of life, when required - Honor Veteran partners by working with Veteran organizations to increase awareness of Veteran specific programs an needs to improve care for Veterans -On-Call Hospice Support -Pain Management -Symptom Management -Medication Coordination -Registered Hospice Nurses Visits/Nurse Aide Visits/Hospice Team Visits/Volunteer Visits -Bereavement Services -Proud participant in We Honor Veterans program -Full time, on-call team SD, Hospice
