Eligibility:
Adults 18 years and older, older adults, and those wiht physical, developmental, or intellectual disabilities.
Fees:
Contracted with CCI. Also accepts private pay once assessment is completed and needs can be met. Private pay rates on tier system (level 1-3). Costs range from $55.00/day for basic assistance, day programming and a noon meal to complete assistance with ADLS including incontinence care, assistance with meals, medication administration and skilled nursing care services (tube feeding, wound care, etc.). and is $65.00/day. Assistance with a bath or shower is not included in the tiered rates but can be an option for an extra $20.00.

Program Description:
Adult Day Center that is fully licensed by the State of Wisconsin and can provide the following services: Therapeutic activities tailored to meet individual needs Assistance with Activities of Daily Living Assistance with showering Lunch and snacks provided with special diet accommodations Full spectrum of care Medication management Structured, safe, and secure environment Featured Programs: Art therapy Music therapy Pet visits Sensory stimulation and integration Live entertainment performances Family involved programming Video games Cooking and baking Gardening Outings Educational programming Focus on personal improvement SD, Respite
