Program Description:
The goal of case management services is to work with clients to help them attain a greater degree of self-sufficiency and overall improvement in their lives. While there are no charges to clients for Outreach and Case Management Services, the program readily accepts donations. The outreach case manager works with clients to identify issues, problem solve, and together create a plan to address long term needs and transform lives. As an advocate for the client, the case manager will help the client obtain available community resources. Spanish speaking Outreach staff are available at all of our primary sites. To schedule an appointment with one of our Outreach Case Managers, please call the primary contact number in your area: Provides Spanish interpreting, translating to non-English or limited English speaking individuals. Provides community outreach information and referral, assists with navigating and understanding educational, judicial and health care systems. - linking in times of crisis to needed services and programs that are available for the following needs: •food and clothing •translation/interpretation •medical/dental care •material resources •counseling services •link to education and job opportunities •link to available community resources Outreach Case Manager available by appointment, at Kenosha sites. Services provided may be advocacy and information and referral to community resources. Home visits may be possible, by appointment if needed. Call for details. SD
