Program Description:
The Children's Community Options (CCOP) program helps children who are living at home who have substantial limitations in multiple daily activities as a result of one or more of the following disabilities: Developmental disabilities (DD), Severe emotional disturbances (SED), and Physical disabilities (PD). The CCOP Program funding can be used to support a range of different services that assist the family in caring for their child with long-term care needs successfully in the family home. An individualized assessment is conducted with the family of an eligible child to determine what services and/or goods are most appropriate and needed to assist the family care for their child. Families without Medicaid or other program supports can apply or who are waiting to be determined eligible. Some examples of services that could be covered with CCOP funding include: -Home modifications -Respite care -Specialized nutrition and clothing -Vehicle modifications -Home training / parent courses -Other goods and services that are specialized but not otherwise covered by other funding sources.
