Program Description:
Provides a broad array of services and support for individuals with a severe and persistent mental illness, that are delivered through an assertive, community-based management approach for adults age 18 and over in a natural supportive service system.
Services are customized in treatment plans, to address each person's needs, identify individual strengths that promote recovery and assist in achieving positive outcomes.

- Case management and counseling
- Medication and symptom monitoring
- Financial management
- Vocation/Employment referral and training
- Psychotherapy
- Housing assistance
- Social and recreational activities
- Community living skills training
- Coordination and Linking
- Assessment and Diagnosis
- Advocacy
- Alcohol and substance use counseling
- Family and community consultation and education
- Learning and performing daily living tasks
- Medical/transportation support
**Behavioral Health Resources include information related to mental health and substance use disorders, life stressors and crises, as well as stress-related physical symptoms. Behavioral health care refers to the prevention, diagnosis, and treatment of these conditions.
