Eligibility:
Serves adults who require supportive residential care and meet one or more of the following criteria: older adults, individuals with developmental disabilities, physical disabilities, dementia or Alzheimer's disease, mental health conditions, or substance use recovery needs.
Admission is based on an assessment of the individual's care needs and our ability to provide appropriate services. Residents must be able to benefit from the adult family home setting and comply with program requirements.

Private Pay. Contracted with Community Care.
Intake Process:
1.) Prospective clients, family members, or referral sources contact Community of Life to inquire about services and schedule a tour or consultation.
2.) Community of Life will do initial assessment is conducted to evaluate the individual's needs, preferences, and eligibility for services.
3.) Community of Life reviews the assessment to determine whether the program is an appropriate fit for the client and can meet their support needs.
4.) If accepted, admission paperwork, service agreements, and any required documentation are completed.
5.) Following admission approval and completion of required documentation, services will begin on the consumer's move-in-date.
