Program Description:
Crabtree Supportive Living provides medication management, transportation to all medical appointments and grocery shopping, money management, 24 hour on call staff alongwith daily staffing checks to ensure proper hygiene, apartment cleanliness and personal communication for health and personal concerns. Trained staff provide support with overall health and well being of each consumer. All Support Services that are provided by Crabtree Supportive Living, are funded through Community Care Family Care, My Choice Wisconsin Family Care, and Kenosha Human Developement Services. or private pay, please call for full details. All monthly rent and living expenses are the client's responsibility. Rent will include all utilities, high speed internet and free laundry access at all Crabtree owned properties. The following activities are available on weekends and are subject to change: YMCA, Special Olympics, Movies, Festivals and other local events. If you want to stay in your current home, our staff will provide the same support services where you reside in Kenosha. SD, Assisted Living
