Program Description:
Crisis Case Management is a short-term crisis stabilization program available to individuals with a mental health diagnosis or who are dually diagnosed. The goal of the program is to assist with transitioning clients to a level of less restriction and assistance in managing symptoms to avoid a placement of great restriction. To achieve these goals, participants are provided with hands on case management, mentoring, individualized crisis planning, assistance with accessing community resources and mental health education with an emphasis on recovery. **Behavioral Health Resources include information related to mental health and substance use disorders, life stressors and crises, as well as stress-related physical symptoms. Behavioral health care refers to the prevention, diagnosis, and treatment of these conditions.
