Eligibility:
Individuals with memory concerns, diagnosis of dementia, or suspected dementia. Family and friends of these individuals. *This service is for those caring for someone living in the community, rather than a facility. Those caregivers living in a facility should contact the ADRC for connections to resources.

Program Description:
This program offers support, education and assistance in finding resources for the caregivers of people with dementia and their caregivers. The person with dementia can not currently be enrolled in a Medicaid Waiver program or be living in as assisted living facility or nursing home. A Dementia Care Specialist can: - Meet with you to discuss your concerns and provide guidance - Connect you to resources that are specific to your planning needs - Help you and your family understand Alzheimer's disease and related dementias - Facilitate support groups - Memory screen - Provide training and suggestions to cope with behavioral changes and to develop skills that can help reduce stress in the home - Offer Music and Memory, a specialized program to help manage behavioral symptoms and improve quality of life.
