To be eligible for EyeCare America, you must meet these criteria: -A U.S. citizen or legal resident -18 years old or older -Not belong to an HMO or a PPO, nor have private insurance nor benefits through the Department of Veterans Affairs -Not seen by an ophthalmologist in three oor more years -Not seen by an EyeCare America volunteer ophthalmologist in the past

Program Description:
EyeCare America is a public service program of the Foundation of the American Academy of Ophthalmology. The goal is to reduce avoidable blindness and severe visual impairment by raising awareness about eye disease and care, providing free eye health educational materials and facilitating access to medical eye care. If eligible, participants will be matched with an EyeCare America volunteer ophthalmologist. SERVICES COVERS: -A medical eye exam, provided at no out-of-pocket cost. -Up to one year of follow-up visits with the volunteer ophthalmologist for any condition diagnosed during the initial medical eye exam. SERVICES NOT COVERED: -Eyeglass or contact lens prescriptions, or the cost of eyeglasses or contact lenses are not covered by the EyeCare America program. If you are concerned about the cost of these items, discuss this with the doctor before the examination and visit our eyeglasses resources page. (EyeCare America provides medical eye exams only.) -Additional services necessary for your care, such as surgical facilities, anesthesiologists, injections, and medications, are not provided by the EyeCare America program. They are your responsibility and beyond the scope of the EyeCare America program. The ophthalmologist is a volunteer who agrees to provide only services within the program guidelines. ADDITIONAL REFERRALS: •If you were eligible for the Program, and require a re-referral to another ophthalmologist for specialty care, you or the EyeCare America volunteer ophthalmologist MUST contact EyeCare America in order to continue receiving care through the program. We may be able to locate another EyeCare America volunteer to provide the care. SD, Eye Care
