Program Description:
My Choice Family Care-Care Wisconsin is one provider option for Kenosha County residents in the Family Care program. Family Care offers a wide variety of long-term care services, making it possible for seniors and adults with physical and/or developmental disabilities to live more independently at home or other community settings. Family Care allows services to be tailored to person's needs, circumstances and preferences, including the option to self -direct parts of their services. Members will continue choosing their own physicians while Family Care provides added long term support.
