Program Description:
Community Care, Inc (CCI) is a Managed Care Organization (MCO) that offers the Family Care Partnership Program. The Family Care Partnership Program offers long-term care services and health services for seniors and adults with physical and/or developmental disabilities, making it possible for members to live more independently at home or other community settings. The Family Care Partnership program integrates a full range of long term, primary, acute and preventive care through the use of interdisciplinary teams. Those that participate with complex health needs will receive comprehensive, continuous, total care that addresses their changing needs. Participants must receive all routine care from a network of healthcare providers. allows services to be tailored to person's needs, circumstances and preferences.
