Froedtert Pleasant Prairie Hospital is one of two hospital campuses for Froedtert South, part of Froedtert & the Medical College of Wisconsin health network. *Provide a copy of a completed Power of Attorney for Health Care to the Medical Records Dept. of the hospital system you normally use. Medical Records Fax Number: (262) 656-2535. Virtual Visit Options For your safety and convenience, many Froedtert & MCW providers offer video visits. This allows you to video chat with a doctor via mobile device, tablet or computer right from home or wherever you are. Video visits are available to new and existing patients and are a secure way to be seen for most health care needs. You can also choose to start your virtual visit right now, or schedule with a provider during normal clinic hours. A video visit is a live, interactive appointment with your Froedtert & MCW primary care provider. These visits are available exclusively through your MyChart account and can be accessed via your mobile phone, tablet or desktop device. Contact yor provide for full details.
