Program Description:
Provides comprehensive medical care in patients' home. Home visits are done with a physician and medical assistant. Most lab and diagnostic testing can be done in the patient's home. Provides all levels of care, from routine exams to complex treatment plans. HarmonyCares physicians provide either episodic consultation or ongoing primary care, whichever is needed. HarmonyCares does not follow the patient in the hospital, will coordinate care and post hospitalization care. SD
