Program Description:
Horizon Home Care and Hospice offers a full continuum of services including:
- Home Health (Nursing, HHA, physical, occupational and speech therapy)
- Palliative Care
- Hospice Care
- Supportive Care (CNA and Homemaker Companion)
- Aging Life Care
- Grief Resource Center
Brief Description of Volunteer Opportunities:
• Comfort Support: Assist behind the scenes in our offices, prepare hospice welcome packets and lap blankets, or volunteer virtually from home.
• Home Hospice: Provide companionship, caregiver respite, or a supportive bedside presence in homes and care settings.
• Musician: Share the gift of music with patients in their homes or partner facilities.
• Pet Therapy: Visit hospice patients with a certified therapy animal.
• Veteran to Veteran: Connect with fellow veterans through shared experiences and companionship.
