Program Description:
Hospice Alliance is committed to providing compassionate physical, emotional and spiritual care and educating patients, and those who support them and the community. Personalized care for patients and families who are facing life-limiting illnesses, is available in the patient's or loved one's home, assisted living and skilled nursing facilities or in the Hospice Alliance House. Hospice Alliance care is delivered through a multidisciplinary team of registered nurses, certified nursing assistants, social workers, chaplain, volunteers and bereavement counselors. Hospice Alliance provides both palliative care and hospice care. - Supportive Care Management (SCM)/(Palliative Care): Supportive Care Management offers an additional choice for people with life limiting chronic or advanced illnesses. SCM is a palliative care program that assists you and you family in making treatment decisions that support the best quality of life possible. This is done by anticipating, preventing, and treating symptoms and suffering that is associated with a chronic illness. ***You may be on Pallative Care (PC) and seek active treatment.*** SD, PP, Hospice Providers
