Program Description:
IRIS (Include, Respect, I Self-Direct) IRIS is Wisconsin's Self-directed Supports Program for older people and adults with disabilities. IRIS participants and their families self-direct the plan components that work best for them, and TMG is available to partner with people for as long as they’re in the program, ensuring that each part of the person’s plan is performing as needed. Through frequent communication, research, in-home visits, goal assessments and plan modifications, TMG’s knowledgeable, compassionate consultants become key allies in achieving long term care goals If you decide you want to participate in IRIS, the Aging and Disability Resource Center (ADRC) will send the IRIS Consultant Agency (ICA) a referral form with information they need to begin the enrollment process. The IRIS Consultant Agency will provide orientation to the IRIS program, work with you to develop plan of supports,and give information needed to understand what it means to self-direct supports within a budget. Ongoing individualized support is available to help people identify and achieve their goals,update plans and assure health and safety.
