Program Description:
The Information and Assistance (IAA) Specialists can help with a variety of concerns. Services can be provided in the ADRC office, over the phone, by email, or in a person’s home. IAA Specialists provide:
-Access and eligibility to publicly funded Long Term Care programs
-Information on a wide range of topics
-Medical Equipment Loan and supply Closet
-Options Counseling
-Youth Transition
Who we serve:
-Adults (18 or older) with a physical or developmental disability, brain injury or any form of dementia
-Family members and caregivers seeking long-term care services
-Older adults
-Youth transitioning (age 17 and 6months) to adult services
Information and Assistance (IAA) Specialists provides unbiased information on a wide range of topics and services. Examples include:
-Caregiving resources
-Food resources
-Home health care
-Housing options
-Medicaid and access to benefit programs
-Senior dining programs
-Social Security Disability application process
-Transportation
- Medical equipment
- Socialization and recreation for seniors and people with different abilities
- Intake for reports to Adult Protective Services
