We serve all patients in Kenosha County, Racine County, Eastern Walworth County and Northern Illinois.

Fee: varies depending on service. Forms of payment accepted: Medicaid, Medicaid-HMO, Medicare, private insurance, and private pay. There is a sliding fee scale based on ability to pay for uninsured patients. There are also discounted fees for upfront self payment for services.

Program Description:
Kenosha Community Health Center is a Federally Qualified Health Center that provides access to quality care through a multi-disciplinary team of Primary Care Medical, Dental and Behavioral Health Providers who collaborate to ensure that each patient has access to high quality, compassionate care. Care teams are made up of Physicians, Nurse Practitioners, Dentists, Dental Hygienists and Behavioral Health providers. Community members are treated regardless of their ability to pay by assisting them in signing up for insurance or discounting services on a sliding scale based on patient/family income. Most forms of insurance are accepted. Services provided at our multiple locations include: Adult Medical Services: - Annual physicals - Chronic disease management - Immunizations - Vaccinations - Preventative care - Care coordination - Family planning, - Gynecologic exams and screenings - STD/HIV testing - Laboratory services Pediatric Medical Services (birth through adolescence) - Well-child checkups - School/Sports physicals - Sick child visits - All age-specific required screenings - Childhood immunization Dental Services (Pediatric and Adult): - General Dentistry - Extractions - Periodontal treatment (gum care) - Endodontic care - Restorations - Dental hygiene (cleanings) - Fillings - Dentures Seal-A-Smile: A children's oral health prevention program sponsored by the Wisconsin Children's Health Alliance. KCHC's Seal-A-Smile program provides the following services and oral health education to children from kindergarten through Middle School - all at no cost to families. - Oral health assessments - Dental sealants - Fluoride applications Behavioral Health Services: - Psychiatry, Psychotherapy and Counseling - Psychiatric medication management - Parent-Child Interaction Therapy - Coping Strategies - Behavioral Screenings and Interventions - Chronic Disease Management - Group Therapy Care Navigation Services - Nurse Care coordination - Behavioral Health & Resource Specialists: Contact information and referrals when needed for Housing, Food pantries and meal sites, free cell phone, adult/children clothing, legal services provided by Legal Aid, Prenatal/Pregnancy resources, referrals to free community classes, energy assistance referrals, smoking cessation information, Lifeline, Meals on Wheels, Support Groups, Referrals for protective services, prescription assistance program, referral to the Well Women's Program, Arranging transportation and applying for or renewing insurance. - Patient Access Representatives: A team of Outreach and Enrollment Specialists who assist patients in enrollment for Medicaid or Marketplace insurance plans and discount services to those without insurance based on their income. Specialists will help with Insurance applications/questions, computer access, informational sessions, and counseling. Pharmacy Services: As a Federally Qualified Health Center KCHC participates in the Federal 340b Discount Drug program and eligible patients may obtain prescriptions drugs at a deeply discounted prices through locally participating pharmacies.
