For FoodShare: You can apply online at ACCESS.wi.gov, by mail, phone, or in person. If you choose to apply by mail, complete the FoodShare Application (F-16019). You can get the application from the Job Center, General Reception or online at https://www.dhs.wisconsin.gov/forms/f16019.pdf If you want to apply by phone, call WKRP at 888-794-5820. If you want to apply in person, call WKRP to make an appointment.

Program Description:
Administers local, state and federal public assistance and emplyment programs and child support services. Note: If you live in Racine or Kenosha County and wish to enroll or have questions about FoodShare(Quest Card), Medicaid (T19), Badger Care Plus, Caretaker Supplement, or Family Planning, please call Wisconsin Kenosha Racine Partners (WKRP) at 1-888-794-5820, Monday-Thursday, 8:00am-4:00pm; Friday 8:00am-12:00pm.
