Program Description:

Offering:
o 24-hour awake staff supervision
o Therapeutic diet management
o Home style meals prepared in-house
o Medication management and education
o Registered Nurse available 24 hours a day
o Individual care management with involvement from outside agencies
o Structured in-house activity program
o Community based program and leisure activities
o Support services such as Occupation Therapy or Physical Therapy, as needed, provided outside of the facility.
o Transportation to all appointments or outside therapies.
o Assistance with ADLs
o In-house laundry facilities
o Assistance: with personal budgeting, assistance obtaining all funding sources available to each individual such as, social security, healthcare, and DVR.
