Program Description:
Answers questions for those who receive benefits through State SSI, SSI HMO Manage Care Program, BadgerCare Plus Program, Food share program and Medicaid (Title 19) including coverage, billing, and prior authorization. Also provides referrals to dentists/dental service providers who accept Medicaid (fee for service).

**Wisconsin Medicaid members must get all medical equipment known as Complex Rehabilitation Technology (CRT) from vendors enrolled in Wisconsin Medicaid as a CRT supplier.
CRT suppliers are experts who can help you get the medical equipment that is the best fit for you and your
life. You may have to get CRT equipment or CRT repairs from a different supplier than you have in the past. Call ForwardHealth Member Services at 1-800-362-3002 or your HMO’s member services for help finding a CRT supplier or access the website for more information:
