Program Description:
The United Community Center is an innovative facility that provides comprehensive and customized services to elderly individuals with physical impediments, Alzheimer's disease, and other dementias. The expansive facility is uniquely designed to provide daily care to the Latino elders of Milwaukee County and employs bilingual and culturally competent staff. The United Community Center also provides screenings and evaluations for memory loss in the Memory Clinic. The center supports primary physicians through diagnostic consultation (English and Spanish), as well as offering programs for families and caregivers, including training and support groups. **Now offering in Kenosha, Mobile Memory Clinic. Vanessa Anciani, Social Worker, is able to conduct virtual meetings with the patient and family caregivers. Please call (414) 649-2808, to discuss case and determine if eligible to be seen by UCC Memory Clinic. A nurse and social worker will conduct a in home cognitive screening, a family/caregiver interview and a home safety assessment. Depending on the results of the initial test, the person could be referred to a Neurologist who will provide further testing to determine a diagnosis and make recommendations for a continuum of care thru the primary physician. Our nurse and social worker are able to conduct virtual meetings with the patient and family caregivers. The clinic neurologist is able to see the patient and talk to the family members via virtual meeting as well as for the diagnostic exam. Patient consent is obtained verbally and we are using a HIPPA compliant tele-medicine platform, under current modified HIPPA regulations. In order to conduct these tele-health assessments, the patient and family caregivers need to have access to a smart phone or computer/laptop with camera and microphone, and a good internet connection. Services •Memory Clinic The Memory Clinic provides comprehensive services to elderly individuals with Alzheimer's disease and other dementia's as well as those with physical impairments. The clinic offers many services to support individuals and their families, including: •Memory screenings •Evaluations by a geriatrician specializing in dementias •Customized treatment •Social support services to both the client and family members •Coordination of care with the client's primary physician •Early Memory Loss Prevention Group The center also supports primary physicians and their staff through outreach, ongoing education, and support services for patients and their families. •Adult Day Center UCC's Adult Day Center provides services designed to meet the needs of those with cognitive impairments such as Alzheimer's and other dementia, those with caregiver needs, and those with physical impairments. The program is structured and comprehensive, providing a variety of health, social, recreational, and support services in a protective setting during the daytime hours. Our goal at the Adult Day Center is to ensure all participants truly enjoy each day by offering a variety of opportunities for them to experience at their own pace. In addition, the Adult Day Center strives to provide support for all caregivers. The daily activities are specially designed to create a "homelike" environment for participants to establish friendships and foster independence and satisfaction. Participants discover previously unrecognized talents and develop new skills through a diverse assortment of activities, which vary daily to allow participants to experience a full and flexible schedule. All services offered are available in both Español and English. The Adult Day Center is located on the 2nd floor of the United Community Center at 1028 S. 9th Street, Milwaukee, WI 53204.
