Program Description:
Provides free medical identification jewelry, informational materials, virtual educational programs and support for families and individuals affected by chronic kidney disease and the professionals who care for them throughout Wisconsin. Provides specialized information and virtual education programs for individuals with a transplant or awaiting transplant.

**Patients with chronic kidney disease, those on dialysis, transplant recipients and living donors are all eligible to receive free medical identification jewelry, for CKD patients, those on dialysis, transplant recipients and living donors. Allow 2-4 weeks for processing. Call for order form.
