Program Description:
Provides free medical identification jewelry, informational materials, virtual educational programs and support for families and individuals affected by chronic kidney disease and the professionals who care for them throughout Wisconsin. Provides specialized information and virtual education programs for individuals with a transplant or awaiting transplant. Free Kidney Health Information Kit To learn more about kidneys, diabetes and blood pressure, request your free Kidney Health Kit. Email: screening@kidneywi.org or call: 414-897-8669 (800-543-6393 toll-free) - Offers free kidney health community programs, online - The NKFW also promotes diabetes-friendly food donations through HealthyShelves.org. **Patients with chronic kidney disease, those on dialysis, transplant recipients and living donors are all eligible to receive free medical identification jewelry, for CKD patients, those on dialysis, transplant recipients and living donors.
