Program Description:
Program of All-Inclusive Care for the Elderly, also known as PACE, is an innovative program that provides a full range of long-term care, health and medical services, and prescription drugs to enable people to live as independently as possible. Offers long term care, which is any service or support which may be needed as a result of a disability, getting older, or having a chronic illness that limits the ability to do the ability to do things throughout the day. This includes things like bathing, getting dressed, making meals, going to work, paying bills and much more. PACE covers your health and medical services of a physician, registered nurse, physician assistant and other qualified medical professional. In addition, Medicaid and Medicare drug services are provided by the Manage Care Organization (MCO) which operates PACE, so no separate Medicare part D drug plan is needed. Provider Directory can be found here: https://www.communitycareinc.org/members/provider-directories
