Eligibility:
Wisconsin resident age 65 or older, Be a U.S. citizen or have qualifying immigrant status; Income determines level of participation. Assets such as bank accounts, property, etc are not counted. *Prescriptions must be written by a physician who is enrolled in Wisconsin Medicaid to be eligible for reimbursement. Program Description:
SeniorCare is a prescription program with three levels of participation dependent on one's income. Prescriptions must be written by a physician enrolled in Wisconsin Medicaid. Only income is counted, assets are not counted. There is an annual $30 enrollment fee. Other eligibility requirements are that one be a Wisconsin Resident and age 65 or older. Levels of Participation: Level 1: For seniors whose annual income is $25,536 for an individual and $34,624 per couple. No deductible or spend down. Prescriptions are $5 for generic and $15 if it is a covered brand name prescription drug. Level 2A: For seniors whose annual income is between $25,537-$31,920 for an individual and $34,625-$43,280 per couple. There is a $500 deductible per person, during this time one will receive a discount off most covered prescriptions. Once the deductible is met, one would pay $5 for generic medications and $15 if it is a covered brand name prescription drug. Level 2B: For seniors whose annual income is between $31,921-$38,304 for individual and $43,281-$51,936 per couple. There is a $850 deductible per person, during this time one will receive a discount off most covered prescriptions. Once the deductible is met, one would pay $5 for covered generic medications and $15 for each covered brand name prescription drug. Level 3: For seniors whose annual income is above $38,305 per individual or $51,937 per couple. Pay retail price for drugs equal to the difference between your gross annual income and 240% of the federal poverty level. This is called "spend down." Covered drug costs for spend down will be tracked automatically. During the spend down, there is no discount on drug costs. After spend down is met, meet an $850 deductible per person. Pay SeniorCare rate for most covered drugs until the $850 deductible is met. After the $850 deductible is met, pay a $5 co-pay for each covered generic prescription drug and a $15 co-pay for each covered brand name prescription drug. SD, Health & Wellness
