Program Eligibility
To be eligible to receive assistance from
any of the TAP program options, you must:
• Be a Wisconsin resident.
• Have a documented hearing loss.
• Purchase from a list of approved
items and vendors.
• Have a household income that is
not more than 200% of the federal
poverty level.
• Have not received TAP assistance
within the past three years.

Note: People who are able to sign the Hearing Loss Certification form include: licensed physicians, certified audiologists, or hearing instrument specialists

Program Description:
Telecommunication Assistance Program (TAP) is a funding program for specialized telephones and related devices. TAP is for any Wisconsin resident who is low income and Deaf, Deaf Blind, or Severely Hard of Hearing. TAP funding is limited and is available on a first come, first served basis.
