Program Description:
Department of Motor Vehicles service center provides driver license, vehicle registration, disabled parking identification, medically impaired driver reports, and Wisconsin State ID's.
- Wisconsin state ID cards: Original (8 years) $28, renewal (8 years) $28, Duplicate $16; for voting purposes (free). When applying for an original ID card you must visit the DMV Service center, complete proper form and provide required documentation. You may apply online for a duplicate or renewal of an existing ID card. All ID cards will be mailed.
- Disabled hang tags: Permanent: No charge ($3.00 counter service fee if purchased at DMV office) Temporary: $6.00, plus counter service fee if purchased at DMV office: Disabled permit questions: 608-266-3041;
- Medically Impaired Driver Questions: 608-266-2327
